The Illinois Estate Planning Packet contains the following Forms:

1) The Illinois Statutory Power of Attorney For Health Care: This form allows you to state your wishes in the event you are not able to make or communicate decisions about your health care and appoint someone to oversee your health care wishes.

2) The Illinois Declaration (Living Will): This form allows you to make the decision as you whether or not you would want to receive life-prolonging treatments at the end of life. 

3) * Illinois Revocable Living Trust - Several options available: The revocable living trust  allows you to determine who will get your property when you pass away.

4)  * Illinois Will - Several options available:  The will sometimes called "a last will and testament" is a document that states your final wishes. It is filed with a probate court when you pass away and the court makes sure your final wishes are followed.
